If you are a new patient and have an appointment scheduled you can save time by printing out our New Patient forms and filling them out prior to your appointment. This may help reduce your wait time in the office and allow you the opportunity to give thoughtful answers to important questions about your health history.
We advise patients to arrive 15-20 minutes early in order to complete any additional paperwork. You will also be required to bring a picture ID such as a valid driver’s license and a copy of your current insurance card.

You (or your responsible party) should bring photo identification and insurance cards to every appointment. Also, it is helpful to bring any previous medical records, include items such as actual x-rays or other imaging studies to help us understand your situation, move your appointment along, and avoid unncessary duplication of medical testing. We also recommend that you have a current list of your medications. For your convenience the registration information can be filled out before your visit.
